Description and Service Details
Description (Service) One of three Long term Care Homes owned and operated by the Region of Halton, Creek Way Village is a three storey facility located off Appleby Line in a residential community. Creek Way offers 24 hour nursing and medical care to 144 individuals with varying disabilities including cognitive impairment. 
 
A private childcare centre is situated at Creek Way Village, which adds to the village-like feel of the facility and provides an excellent opportunity for inter-generational programs.
Hours Office Mon-Fri 8:30 am-9 pm; 
Home operates Mon-Sun 24 hours

Eligibility determined by your Local Health Integration Network - Home and Community Care or case manager * client must have a valid health card, have maximized available community supports, have care requirements that can be met in long term care
