| Description | Halton Region Services for Seniors offers a Supportive Housing Program and a Supports for Daily Living program which provide personal support by qualified staff. Wellness promotion is a key component of the program. The goal is to provide support for seniors to encourage long-term, independent living in their own apartments in supportive housing communities. Services include * personal care and support with bathing, hair care, dressing, medication, exercising, reassurance, safety checks * homemaking support with light housekeeping, laundry, meal services. Professional services (e.g. nursing) are provided through the Community Care Access Centre. The Support Services Coordinator for the Supportive Housing Program, the Supports for Daily Living Program Coordinator and the CCAC Case Manager work closely together to ensure that the seniors' needs are met. Seniors housing projects where the Supportive Housing Program is available to eligible applicants: * Bonnie Place I and II, 500 Claridge Road, Burlington * The Bruce Apartments, 40 Ontario Street, Milton * Wellington Terrace, 410 John Street, Burlington Supports for Daily Living A new Supports for Daily Living program opened at John R. Rhodes Senior Citizens Residence, 271 Kerr Street, in Oakville in August 2009. Halton Region provides this service to the residents of John R. Rhodes Seniors Residence and eligible individuals living in the surrounding Kerr Street neighbourhood. | ||

| Eligibility | Ages: 60 year(s) and up Senior residents who require some form of every day living assistance may be eligible for the program. Applicants must be able to direct and participate in their own care; 24 hour supervision services are also available. | ||
