Description and Service Details
Description (Service) An individualized, six month exercise and educational program to help people recover from heart attacks, heart surgery, to improve cardiovascular fitness and to reduce risk factors for coronary artery disease * most patients begin the program within 6-12 weeks post hospitalization

This program:
  • is for people who have had some type of heart disease such as angina, heart attack, congestive heart failure, heart surgery, or angioplasty
  • includes medical evaluation, graded exercise testing, individually prescribed exercise programs, risk factor education, and counseling
  • multidisciplinary team including cardiologists, nurses, physiotherapists, dietitian, kinesiologists, social workers, trained exercise supervisors, and volunteers
  • provides education and activity guidelines for patients to utilize between hospital discharge and beginning the outpatient program
