| Description | Consultation on managing multiple medications for a chronic condition * interpreting different instructions * understanding potential side effects and drug interactions * how to get the most from your medications MedsCheck Program: * a free 20 to 30 minute one-on-one annual meeting with your pharmacist to ensure that you are safely and appropriately using all types of medication * discuss how your prescription, over-the-counter and alternative medications may be affecting each other MedsCheck for Diabetes: * for anyone who is diagnosed with type 1 or type 2 diabetes * all Ontarians who have diabetes are eligible, whether or not they are taking prescription medication * an opportunity for the pharmacist and the patient to review medication including advice, training on devices, monitoring and diabetes education * a one-on-one consultation with your pharmacist towards overall disease management MedsCheck at Home: * for patients taking a minimum of three chronic prescription medications who are not able to visit their pharmacy because of physical and/or mental health condition and/or distance to a community pharmacy * the pharmacist will visit the patient's home for a one-on-one consultation and assessment * includes a medicine cabinet clean-up and the ability to remove unused medication for proper disposal MedsCheck for Long-term Care Home residents: * quarterly medical reviews and an annual in-depth medical analysis conducted by the pharmacist in the Long-term Care Home Objectives include: *promoting healthier patient outcomes and better care * improving and optimizing drug therapy for residents *promoting interdisciplinary collaboration in patient care * quarterly reviews should include medication selection, dosage, hours and route of administration, duration of therapy, treatments, allergies and drug reactions * reviews will identify any possible drug related problems that may require a more in-depth therapy analysis (annual review) and follow-up in collaboration with the patient, caregiver and health care team To learn more about MedsCheck talk to your pharmacist or call the INFOline at 1-866-255-6701 or visit MedsCheck. | ||||||||
