Description (Service) | Provides case management supports to youth who are struggling with their mental health and/or substance use. In addition, provide housing services to youth at risk of homelessness. Transitional Aged Youth (TAY) Coordinators provide: * activities of daily living * supportive counselling * advocacy * life skill training * connection to community services and referrals * access to our 24/7 crisis telephone support * support in strengthening relationships with community and family, addictions issues and minor involvement in the criminal justice system |
