Description and Service Details
Description (Service) An education and early intervention program that utilizes community referral sources to identify seniors living in severe self-neglect known as Diogenes Syndrome. 
Seniors identified as living in self-neglect are contacted and offered support through: 
* case coordination  
* multi-disciplinary intervention  
* advocacy  
* problem solving  
* long term monitoring
Hours Mon-Fri 8:30 am-4:30 pm
Areas Served Acton ; Burlington ; Campbellville ; Georgetown ; Halton Hills ; Milton ; Oakville
Eligibility Ages: 49 year(s) and up 
* existence of severe self-neglect or hoarding issues 
* isolated in the community 
* multiple service needs requiring coordination and follow-up 
* severe conditions or handicaps compromising functional capacity including cognitive or physical impairments with confusion and sensory losses 
* mental health issues
