Public Comments | Complaints process for services provided through Local Health Integration Networks: The client's Case Manager (for home care) or Placement Coordinator (for Long-Term Care placement) is required to explain the following review procedure: 1) client may request review by a Manager 2) If still unresolved, client may ask for review by the LHIN Ombud 3) if dissatisfied with the decision of the Ombud, client may appeal to the Health Services Appeal and Review Board (for contact information see separate listing here) This process may also be supplemented by reporting complaints to the Long-Term Care Action Line (for contact information see separate listing here) * Action Line can listen to concerns and mediate disputes between clients and their LHIN and/or service provider, but has limited authority to make decisions on services |

Description (Service) | Plan, integrate, and fund local health care * deliver and coordinate health care support at home, at school, or in the community * care coordinators work with people in the community or leaving hospital to determine what help is needed and arrange for care * care is provided in community settings such as the home, school, residential facility or HNHB LHIN community clinic * referral to other community services when appropriate Services available to eligible patients may include: * nursing * nurse practitioner * personal support * physiotherapy and occupational therapy * speech language therapy * social work * nutrition and food counseling (dietitians) * end-of-life care * medical supplies and equipment (patient-specific supplies for HNHB LHIN care such as catheters, ostomy bags, or wound care supplies) * access to long-term care homes * access to supported living (adult day programs, supportive housing, retirement homes, and assisted living) * health support in the school * specialized services for people living with specific health conditions * referral to other community supports * Burlington Nursing Care Centre Information and Referral offered through My Health GPS, 1-800-810-0000 (available Mon-Sun 8:30 am-8:30 pm): system navigation * access to community resources * assistance with referrals |
