Description (Service) | Services altered in response to COVID-19 (Coronavirus) * GraceHouse, LifeHouse and Chartwell residences are remaining staffed though closed to visitors * Community and peer individual and group supports will work remotely to support clients and community partners via phone support, email and other virtual solutions until further notice * Our offices are also currently closed to the public * We will continually reassess our support planning and will provide updates as needed * We are committed to continued community service. ----------------------------------------------------- Support coordinators provide support and services based on the person's level of need. Rent-geared-to-income supportive housing options: * transitional housing leading to independent living * collaborative shared housing * shared townhouse living * individual apartments Ready4Life: * ongoing support and housing services to youth at risk ready to transition into the adult system Mental health Community Support: Staff assist with: * case management * advocacy * life skills training * supportive counselling * activities of daily living * access to community services * 24/7 access to telephone support for program participants Programs & Services to support mental wellness: * TEACH, a peer led and run initiative for people living with mental health AND substance use challenges * Anxiety, Self Esteem and Concurrent Disorders workshops * Wellness Recovery Action Plan (WRAP) workshops * Checkpoint, a peer led drop in session providing support in a group setting * harm reduction programs including Evolution, supporting people who are living with mental health and substance use challenges |
