Description (Service) For a full list of COVID-19 symptoms, please refer to the Ontario Ministry of Health website, here 
Individuals with symptoms associated with COVID-19 are asked to self-isolate 
If you think you have COVID-19 symptoms or have been in close contact with someone who has symptoms associated with COVID-19, you can be assessed by a health care provider by: 
* contacting your primary care provider 
* visiting an assessment centre (appointment required) 
Assessment Centre: 
* clinic where people will be assessed by a health-care provider and tested for COVID-19 if required  
* the test involves taking a sample from the throat or nose with a swab  
* you must have an appointment 
* residents of Oakville, Milton or Halton Hills, call 905-203-7963 to book an appointment at a Halton Healthcare Assessment Centre 
* residents of Burlington, see Joseph Brant Hospital COVID-19 Assessment Centre 
You can also use the Ontario Government’s COVID-19 online self-assessment tool or contact Telehealth Ontario at 1-866-797-0000 to determine if you need to seek further care
