Supports for older adults, and adults with disabilities * helps people remain independent in their own homes for as long as possible

Services include:

Dementia Care:

  • one-on-one caregiver support
  • caregiver education sessions
  • adult day program (2 locations: Burlington and Oakville)
  • memory care (respite, home safety assessment, on-going assessment of care needs)
  • memory visiting (trained volunteers provide companionship, some respite)

Home Care:
  • nursing (visiting, shift, specialty, and clinics) care
  • personal care (help with bathing and personal hygiene)
  • overnight care
  • meal preparation
  • housekeeping and laundry
  • companionship
  • help with getting to appointments, errands, grocery shopping
  • Philips Lifeline (personal medical alarm and response)

Hospice and Bereavement Support - Psychosocial emotional support, companionship, respite care, spiritual care for people and their family/caregivers dealing with a life-threatening illness * services provided at home

Tele-Touch - A telephone reassurance program where volunteer callers phone lonely, isolated or frail older adults and adults with limited mobility, providing friendly conversation, security and reassurance * volunteers call according to a pre-arranged schedule * presentations to community groups * volunteer opportunities available (apply online)
