Description and Service Details
Description (Service) Offers time-limited treatment and management for individuals whose diabetes and related conditions require a more intensive and comprehensive team approach * assists patients with prolonged diabetes and multiple co-morbidity, such as organ failure, frequent hyper or hypoglycemia, foot and skin problems, mental health issues and inability to cope * these co-morbidities can cause worsening of blood glucose control and lead to frequent hospitalization and emergency visits * team consists of nurse practitioner, registered nurse, registered dietitian, social worker and pharmacist

Services provided:
  • patient assessment, education, treatment and  management using a patient centered model
  • frequent follow-up and monitoring
  • case management and care navigation
  • collaborate with all providers (specialists, primary care providers, LHIN - Home and Community Care, mental health, etc) involved in patient care to provide the best care
  • communicate regularly with all care providers regarding patients care plans
  • provide expedited psychiatric consult and wound care referrals
  • kinesiologist appointments
