COVID-19 Proof of Vaccination Printing

Public Comments Beginning January 4, 2022, you will be required to use your enhanced vaccine certificate with official QR code. Vaccine receipts without a QR code will no longer be accepted as valid proof of vaccination. 
As of September 22, 2021, you will need to provide your proof of vaccination, your active medical exemption or your participation in a Health Canada approved COVID-19 vaccine trial, along with your valid ID to access some businesses and settings 
* personal identification with name and date of birth that matches the proof of vaccination 
* proof of vaccination is your vaccine receipt or enhanced vaccine certificate with a QR code, sometimes called a vaccine passport or vaccine record

Downloading your Proof of Vaccination

* provincial portal is available for any individual to download their proof of vaccination (enhanced vaccine certificate) - 
* for any doses received in Ontario, regardless of where you were vaccinated 
* if you do not have a photo health card, would like to have your enhanced vaccine certificate emailed or mailed to you, or need assistance, call the Provincial Vaccine Contact Centre at 1-833-943-3900 (available 8 am-8 pm, 7 days a week)
