Description and Service Details
Description (Service) Provides transitional support services in Housing, Life Coaching, Life Skills Training and Mentoring, to vulnerable and racialized youth aging out, or recently aged out of the child protection and foster care system 
 
Youth Life Coaching 
* strength-based, small-group, 12-week program 
 
Moving on With Purpose 
* LifePlan Coaching Service that supports youth aging out of the Child Protection and Foster Care System to build knowledge and capacity needed to transition successfully into independence and competence 
* an 8-week curriculum based learning, delivered in small group sessions and provides youth with individualized attention, structure and accountability 
* youth preparing to age out of care between ages 16-18 can self refer or be referred by community agencies, caseworkers or interested individuals 
 
Housing Support Services 
* program model adheres to the Foyer Housing Model 
* accompanying youth with housing search and helping with applications 
* Youth-Landlord mediation when needed 
* moving and settling-in support 
* linking youth to community resources for employment, health and appropriate financial support 
* Life Skills training to strengthen capacity to live independently 
 
Housing Employment Mentoring (H.E.M.) Initiative 
* focuses on helping youth access and retain transitional housing, employment in high-earning careers and culturally responsive mentoring 
* commitment is to increase successful exits from homelessness and reduce instability on their paths to success 
 
OASIS Home 
* a spacious house, in an accessible location in the North-East end of Oakville, for black male youth (18-21yrs) from the Halton/Peel Region 
* safe, stable accommodation and wrap-around support services to help overcome barriers, thrive and transition successfully into independent adult living 
* designed to duplicate a healthy, supportive family environment 
* engages a strength-based approach in building individual capacity for success 
* provides employment readiness, connection to community resources and mentoring support
