Description (Service) | Services altered in response to COVID-19 (Coronavirus) * Peer-led groups and peer navigation services are working remotely to support clients and community partners via phone support, email and other virtual solutions * Community Supports continue to provide one to one and face to face visits * Offices are currently closed to the public * Grace House, Life House and Chartwell House residences remain staffed, though closed to visitors If you have any questions, please call 1-833-845-WELL (9355) ----------------------------------------------------- Support Coordination for Adults * responds to the needs of adults who experience mental health and/or substance use issues in Halton * offer support coordination, housing, peer support, harm reduction, navigation, and distress/crisis support * supports include a Justice Team that specializes in supporting the unique needs of individuals that have mental health and/or substance use issues and are justice-involved Support Coordination for Youth * responds to the unique needs of youth, aged 16-24, in Halton * offer support coordination, housing, peer support, harm reduction, navigation, distress/crisis support, and support moving into adult services as needed TAY Service Coordination (Transitional Aged Youth) * multi-agency committee that problem solves and assists youth who may benefit from an enhanced service plan and to successfully bridge youth into adult services |
