* person-centred service delivery based on the client 
* support with recovery goals 
* the right to privacy and confidentiality 
* 24/7 access to crisis line support 
 
SHH currently offers the following housing support programs, all of which have varying levels of supports attached to them:  
 
Chartwell Residence 
* located in Oakville 
* accommodates 7 people, 1 year maximum intended 
* affordable rent 
 
LIFEhouse (Living Independently For Experience) 
* located in Acton 
* accommodates 5 people, 1 year maximum intended 
* ages 16 to 30 
* affordable rent 
 
Grace House Residence 
* Transitional Housing  
* located in Oakville  
* accommodates 10 people, 1 year maximum intended 
* affordable rent 
* staff on site 24/7, no phone calls or visitors before 8:30am or after 9:30pm 
 
Townhouses 
* 7 townhouses in total 
* 2 in Acton, 4 in Oakville, 1 in Burlington 
* tenants learn to live together in partnership with other housemates 
* based on rent-geared-to-income 
 
One-Bedroom Apartments 
* approximately 80 apartments within Halton 
* located in Oakville, Burlington, Milton, and Acton 
* based on rent-geared-to-income 
 
ASH Program 
* available to those in the early stages of addiction recovery, who are homeless, at risk of becoming homeless or marginally housed, not experiencing a serious or persistent mental illness, and are at least 16 years of age 
* clients are housed throughout Mississauga and Halton and are provided with intensive community supports in a partnership with ADAPT
Hours Administration: Mon-Fri 8:30 am-4:30 pm 
 
Residence: Staff provide flexible hours of support 
* Emergency telephone support available 24/7
Areas Served Halton Region ; Preference given to those residing in Halton Region
Eligibility Ages: 18 year(s) and up 
 
Chartwell, Grace House, and LIFEhouse residents must:  
* be able to actively participate in day program and household responsibilities 
* could safely evacuate the home independently in case of emergency
Application Admission by referral only 
* referrals are accepted from community agencies, professionals, psychiatrists and local hospitals, educational facilities, doctors, through one-Link referral source (form downloadable from the website)  
* telephone or personal contact must be followed by the referral in writing from appropriate social agency 
* specific application and referral forms will be forwarded once one-Link referral received
