Description (Service) The mandate of the LHIN, within its specific geographic area, includes local planning, community engagement, service integration, improved accessibility, improved financial efficiencies and accountability * also deliver and coordinate home and community care support services

Plans, coordinates, integrates, funds and monitors local health services including:
  • hospitals (Trillium Health Partners and Halton Healthcare)
  • community health centre
  • long-term care homes
  • mental health and addiction agencies
  • community support service organizations
