The Client Coordinator assesses socially-isolated older adults to attend the Circle of Friends program.  By supporting the purpose of the Circle of Friends and engaging with potential participants, the Client Coordinator will make a significan impact on the lives of seniors living in social isoloation and the advancement of community awareness.

Duties

The Client Coordinator selects socially-isolated older adults to attend the Circle of Friends program in accordance with program guidelines and goals.  The Client Coordinator conducts an in-home assessment of potential program participants.

Responsibilities:

  • comply with the Client Agreement and In-Home Assessment procedures
  • accept referrals from sources in the community and conducts an in-home assessment
  • outline the program, privacy, and confidentiality policies with the program participant
  • ensures the In-Home Assessment Form and Client Agreement Forms are completed accurately
  • follow privacy, due diligence, and reporting procedures
  • comply with screening requirements
  • provie ongoing program support
  • other responsibilities as required

  • knowledge of program goals and awareness of the psychological impact of social isolation in a senior population
  • ability to communicate with an older adults who experiences social isolation
  • ability to gain the confidence of the client
  • ability to explain effectively how Circle of Friends will benefit the client
