Description (Service) | COVID-19 -- Emergency shelter remaining open, 24-hour service provision and care * no walk-ins, assessment by phone screen * Outreach Team & Counselling -- supports for wellness, basic needs and food security * doesn't provide transportation * health screening questions prior to visiting * counselling available by phone to youth in need of additional supports for wellness * community outreach, preventative, and educational programming available virtually * in person and external service provider programming, community events are cancelled ---------------------------------------- Emergency shelter, transitional housing (up to 3 months), and community outreach programs, including family reconciliation counselling, and housing search and landlord negotiation assistance * volunteer opportunities Emergency shelter -- crisis counselling and case management * referrals services, including to ongoing clinical support and counselling * access to resource room Family Reconciliation Program -- mediation and counselling for youth who are homeless or at risk of homelessness, and their identified family Home for Good -- intensive case management supporting youth to obtain and maintain permanent housing * collaboration of community organizations using Housing First for Youth model Housing Program -- intensive case management * assists youth in building positive relationships with landlord and/or property management * advocacy and information on rights and responsibilities HYPE Program -- works with youth who are homeless or in precarious living situations * transitional program to prepare for independent living, long term housing and/or to return home, includes life skills and personal development training * capacity 6 Outreach -- assist youth in community in development of life skills, problem solving skills * promotion of social support networks and personal independence Resource Room -- supports development of skills, housing searches, employment and education * tools to develop new skills, build confidence, and find support network |
