Description and Service Details
Description (Service) Support coordinators provide support and services based on the consumer's level of need.  
 
Rent-geared-to-income supportive housing options: 
* transitional housing leading to independent living  
* collaborative shared housing  
* shared townhouse living  
* individual apartments 
 
Mental health community support programs: 
* case management  
* advocacy  
* life skills training  
* supportive counselling  
* activities of daily living  
* access to community services  
* 24/7 access to telephone support for program participants 
 
Ready4Life
* ongoing support and housing services to youth at risk ready to transition into the adult system
Hours Administration: Mon-Fri 8:30 am-4:30 pm

Eligibility Ages: 16 year(s) and up 
 
Have a mental health issue and/or problematic substance use * youth at risk and/or vulnerable within our community  
 
Supportive Housing clients:  
* benefit from having additional supports with housing  
* must have a source of income to meet financial requirements  
* be willing to contribute to a safe, secure healthy living space for all residents and /or neighbors  
* ability to live independently or have the appropriate supports in place to live independently
Application Referral from psychiatric professional, family physician and other agencies, or self, through one-Link application process
