Description and Service Details
Description (Service) The Halton office is the contact point for families/individuals who are interested in accessing:  
* residential services and supports 
* caregiver respite 
* community participation supports (with social and recreational activities, work activities and volunteer activities, including Passport) 
* specialized services, such as service coordination or clinical services  
* person-directed planning supports
Hours Mon-Fri 9 am-4:30 pm

Eligibility Ages: 18 year(s) and up 
individual must have a documented developmental disability (documentation accepted is a psychological assessment) and meet age and residency requirements
Application Documents needed to complete an application: 
Document confirming age: 
* copy of a birth or baptismal certificate 
* passport 
* drivers licence 
Proof of Canadian citizenship, landed immigrant status or permission to stay by Citizenship and Immigration Canada: 
* copy of passport 
* naturalization certificate  
* immigration documents 
* Minister's permit 
Documents confirming Ontario residency include but are not limited to: 
* rental or lease agreement 
* statement of direct deposit for Ontario Disability Support Program 
* employer record (pay stub or letter from employer on company letterhead) 
* mailed bank account statements (not automated teller receipts or bank books) 
* utility bill
