| Description | The Teen Clinic is structured to serve the teens' unique health-care needs. * teens can attend the clinic alone or with a friend, parent, guardian or other family member * at the clinic the health-care provider will first talk with the teen and then the parent or guardian * the teen will then have an opportunity to speak to one of the health-care providers alone The Teen Clinic is made up of: Young Families Program Eating Disorders Program Substance Abuse Day Treatment Outreach Program SickKids Interprofessional Team for Youth (SITY) Good 2 Go Transition Program A program to prepare all youth with chronic health conditions to leave SickKids by the age of 18 years with the necessary skills and knowledge to advocate for themselves (or through others), maintain health-promoting behaviors and utilize adult healthcare services appropriately and successfully. | ||
