| Description | Summit Housing & Outreach Programs, a non-profit registered charitable organization, provides flexible residential and community support services for adults who have an identified psychiatric illness. These supportive programs have been designed to meet the varied needs of the people served. Several rehabilitative supportive models are utilized within the scope of the program. Summit Housing & Outreach Programs is also the lead agency for the Halton Homes Program which provides supportive housing to people with serious mental illnesses who are homeless, or at risk of becoming homeless. Major Services Include: * Supportive housing programs are provided in a variety of housing options (e.g. apartments & shared accommodation) throughout Halton which meet a variety of needs. Staff assist in skills development (budgeting, cooking, socializing, etc), goal setting and utilization of community supports. * A community case management outreach service for people living in their own homes or involved with other agencies in the community * Summit Assertive Community Treatment Team: a client-centered, recovery-oriented mental health service delivery model for persons who have the most serious mental illnesses and have not benefited from traditional out-patient programs. |
