Description and Service Details
Description (Service) A community outreach program, serving residents of Halton. 
* also known as "Hospital without walls," the program is organized as a mobile interdisciplinary team that provides round-the-clock clinical treatment, rehabilitative, and support services to individuals with a severe and persistent mental illness, in their homes 
 
Services provided by PACT include:  
* comprehensive case management with a rehabilitation focus 
* crisis assessment and intervention 
* symptom assessment and management 
* administration and monitoring of medication 
* activities of daily living 
* social and leisure skills development 
* supportive counselling 
* education 
* consultation 
* support to caregivers
Hours Office: Mon-Thu 8:30 am-7 pm * Fri-Sun 8:30 am-4:30 pm

Eligibility Ages: 18 year(s) and up 
 
Services are provided for individuals who: 
* have a history of severe and persistent mental illness, as demonstrated by high hospitalization days, particularly those with a diagnosis of schizophrenia  
* exhibit significant functional impairments due to their mental illness in the areas of self-care, productivity, and leisure 
* continuously require high service needs in excess of 8 hours per month
Application Referrals to the PACT program are accepted from psychiatrists, family physicians and other mental health professionals
