| Description | A non-profit organization providing: * specialized residential services, care and support to individuals over 18 years of age with multiple developmental disabilities * respite services for children and adults * residential services are located in a central administration and recreation building in downtown Oakville * also operate four community homes Services include: * individuals are served by a specifically trained multidisciplinary team consisting of direct care counsellors and other professionals including physiotherapist, speech therapist, occupational therapist, psychologist, nutritionist, nurses, physician and psychiatrist * ongoing assessment, treatment and management * opportunities for basic life skill development, workshop activities and for some supported employment * a ten bed activity-based respite program for children and adults with complex needs * manages Halton Support Services * Dual Diagnosis Service: provides consultation, comprehensive assessment, treatment planning, interpretation, on site support and follow up, mentoring and education/training * Short Term Assessment and Treatment Environment: for those with a dual diagnosis - assesses the individual and develops a treatment plan that will assist in integrating the individual back into their community *Central West Region Passport Agency: administers the program that provides adults with a developmental disability who have completed school and are seeking community participation supports * Central West community Network of Specialized Care: working with Central West Region community partners to develop, coordinate and streamline access to specialized resources An elected volunteer board of directors is responsible for the governance of Central West Specialized Developmental Services. | ||
