Description (Service) Major services: 
Adults (18+): 
* provides service coordination  
* planning/goal setting  
* short term and crisis counselling

* assistance in finding accommodation  
* provide referral and assistance in accessing community resources and supportive counselling  
* assist with coordination and future planning  
* self referral only 
Children (up to age 18): 
* provides assistance to families for information, referral and accessing services

* delivery of the Special Services at Home Program 
Respite Program 
Critical Service Funding: 
* community crisis funds available to assist families, agencies and individuals requiring short term support to prevent or assist the in a crisis situation when other resources have been exhausted

* initial contact point for families and individuals who are interested in accessing residential and non-residential supports 
Halton Wraparound & Seniors Wraparound
