DescriptionResidential placement for youth age 15-18 years old who are not able to live with their family but are not yet ready for independent living. Follow-up support and assistance for former residents of CYP is offered. 
 
Major Services:  
Residential - eight bed, co-ed group home for adolescents * if family re-integration is not possible, the adolescent is prepared for independent living or another alternative. 
 
Non-Residential - community consultation service for adolescents and their families who are experiencing difficulties. Provides * referral * case-management * community liaison * on-going support to former residents. 
 
Transitional Support - provides support to older teens who are moving from residential placement to the community or to adult services. 
 
HoursOffice: Mon-Fri 9 am-5 pm, or by appointment; 
Staff on call 24 hours.
EligibilityAges: 15 year(s) - 18 year(s) 
 
Must be attending school. Preference given to Halton residents.
