Description and Service Details
Description (Service) A non-profit, community-governed organization that provides primary health care, health promotion, illness prevention and community development services which focus on addressing and raising awareness of the broader determinants of health such as employment, education, environment, social isolation and poverty. 
The interdisciplinary team includes doctors, nurse practitioners, nurses, dietitians, health promoters, community outreach workers, outreach social workers, outreach nurses, therapists, and community health workers among other volunteer health professionals such as the staff dentist, dental hygienists, and dental assistants and chiropractors.
Hours Office Mon-Thu 8:30 am-4:30 pm * Fri 8:30 am-12 noon 
Urgent Service Access Team Mon-Fri 11:30 am-7:30 pm
Areas Served St Catharines
Boundaries Generally clients reside in St Catharines, however, there are exceptions for clients who are part of the special programming such as the Migrant Agricultural Worker Program clients Rainbow Niagara and Urgent Service Access Team. Please contact for further clarification.
Eligibility Generally clients of Quest CHC reside in St Catharines and are not registered with another primary care provider. However, there are exceptions for clients who are part of the special programming such as the LGBTQ population, USAT and MAWP clients.  
Quest CHC priority populations are:  
* Individuals experiencing mental health, addictions and or concurrent disorders 
* Street involved populations (homeless, under-housed, sex trade workers) 
* Isolated seniors 
* Sexually and gender diverse populations 
* At risk children and youth and families 
* Individuals who frequent the Emergency Department for care/Opioid dependent  
* Migrant agricultural workers
