Description (Service) The program includes a wide range of inpatient and outpatient services * also provides rehabilitation to patient care units within the Rehab and Geriatrics Program, rehabilitation professionals provide therapeutic services for all other HHS patient programs

Outpatient services:

  •  Acute Hand Program * 905-338-4613 (OTMH) or 905-876-7022 (MDH) * for complex hand injuries and post-op cases requiring treatment from physiotherapists and occupational therapists with specialty training
  • NeuroRehab Step Up / Assess and Restore Program * 905-338-4367 * multidisciplinary outpatient that provides physiotherapy, occupational and speech language therapy for clients who have neurologically * helps maximize cognitive ability, perception, communication, balance, coordination, strength, and functional ability * beneficial for those recovering from a stroke or Acquired Brain Injury, or neurological diseases including Parkinsons, Brain Tumor, Guillain-Barre Syndrome and Multiple Sclerosis
  • Cardiac Rehabilitation and Education Program * 905-845-2571 ext 6653 (OTMH) or 905-878-2383 ext 7274 (MDH)  * designed to improve cardiovascular fitness and reduce risk factors for coronary artery disease
  • Comprehensive Spasticity Management Clinic * 905-845-2571 ext 3545 (OTMH) * provides treatment for adults experiencing muscle spasticity
  • Falls Prevention Clinic * 905-338-4362 * for adults over 65 years of age who have experienced a fall or are at risk for falls
  • Geriatric Assessment Clinic * 905-338-4362 (OTMH) * 905-878-2383 (MDH) * provides comprehensive assessment, a plan for management of geriatric issues and follow-up for geriatric outpatients
  • Urgent Geriatric Assessment Clinic * 905-338-4362 (OTMH & MDH) * provides timely comprehensive geriatric assessments for patients seen in the Emergency Department but not requiring admission and inpatients ready for discharge, who require a timely comprehensive geriatric assessment * goal is to identify and provide a plan for the management of geriatric issues to prevent further Emergency Department visits, avoid unnecessary hospitalizations and reduce length of stay
  • Geriatric Medical Outreach Program * 905-338-4362 * provides assessment and recommendations to enable individuals to age in place * collaborative practice with Trillium Health Partners
  • Physiatry Clinic * 905-845-2571 ext 3545 * provides outpatient consultation for musculoskeletal and neurologic disorders amenable to rehabilitation
  • Prehab Program * 905-338-4613 (OTMH) * for elective hip and knee replacement surgical patients * pre-op training and education prepares patients for surgery, post-op expectations, self-management and coping skills
  • Psoriasis Clinic * 905-338-4613 (OTMH) * 10-month per year service to treat psoriasis and other skin conditions
  • Pulmonary Rehabilitation and COPD Program * 905-845-2571 ext 4613 (OTMH) * 905-878-2383 ext 7695 (MDH) * 905-873-0111 ext 8112 (GH) * doctor referral is required * provides outpatient programming for patients with COPD two days per week, each lasting 2.5 hours * trains patient to carry on a home program once they have completed the 10 week session
  • Speech Language Pathology (Georgetown Hospital only) * 905-873-0111 ext 8112
  • Work-Fit Total Physiotherapy * OHIP-covered services for physiotherapy and occupational therapy * help patients, including those with conditions such as low-back pain, fractures, arthritis, sports injuries, and neurological conditions by providing services that relieve pain, strengthen muscles, improve mobility, restore function and prevent or limit further physical disability * 905-338-4613 (OTMH) or 905-876-7022 (MDH) or 905-873-0111 ext 8112 (GH)
