Description (Service) Supports for older adults, and adults with disabilities * helps people remain independent in their own homes for as long as possible 

Services include:

Alzheimer Services:
  • One on one caregiver support
  • Caregiver education sessions
  • Adult Day Program (2 locations: Burlington and Oakville)
  • Memory care (respite, home safety assessment, on-going assessment of care needs)
  • Memory visiting (trained volunteers provide companionship, some respite)

Home Health Care:
  • Nursing (visiting, shift, specialty, and clinics)
  • Personal care
  • Help with getting to appointments, errands, grocery shopping
  • Philips Lifeline (personal medical alarm and response)
