Description (Service) | Medical care team focused on family and community health * coordinates patient care in order to assess and treat injuries or illness * team includes family doctors, psychologist, registered nurses, nurse practitioners, mental health workers, dietitian, health educator, speech language pathologist and pharmacists Chronic Disease Management Programs: Diabetes, chronic obstructive pulmonary disease (COPD), asthma, heart function program - sessions to learn ongoing self-care * includes information about coping, health guidelines, medication usage, working with health professionals, and informed treatment decisions Mental Health Services - Offered to patients that are unable to afford private assessment or treatment services and do not have access to an employee assistance program or other insurance related options Lifestyle and Wellness - Provide patients with the knowledge and skills to better manage and control of their health Smoking Cessation Counseling - Help to quit smoking, available for Ontario Drug Benefit recipients Speech and Language Services - Help patients with speech and language difficulties, and cognitive-linguistic difficulties Nutrition Services - Nutrition counseling and education to both individuals and groups Cognitive Assessment Program - Open to patients that are medically stable and over the age of 65 for screening purposes only Wellness Seminars - Drop-in sessions on healthy eating, exercise, chronic disease prevention, and overall health and wellness for older adults * free weight and blood pressure monitoring on a monthly basis |
