| Description (Service) | Medical care team focused on family and community health * coordinates patient care in order to assess and treat injuries or illness * team includes family doctors, nurse practitioners, registered nurses, physician assistant, registered dietitian, and other allied health professionals such as mental health counsellor, respiratory therapist, chiropodist, psychologist, psychiatrist * focuses on chronic disease management, disease prevention and health promotion * all appointments with allied health professionals are booked through family physician's office Programs and services:
