Description and Service Details
Description (Service) Specialized service and treatment for individuals who have behaviours related to a diagnosis of dementia. Within St. Joseph’s Healthcare Hamilton provide service to the regions of Niagara, Brant and Halton, Local Health Integrated Network (LHIN) 4. 
Goal of the program is to return clients to an appropriate care setting within their home community after assessment and development of a treatment plan.
Hours Inpatient unit: Mon-Sun 24 hours Community-based programs: business hours Mon-Fri
Areas Served Brant County ; Haldimand County ; Halton Region ; Hamilton (City of) ; Mississauga (City of) ; Niagara Region ; Norfolk County
Eligibility Ages: 65 year(s) and up 
Those seniors who are under the age of 65 who have an age related cognitive decline are eligible for service. Referring sources are pleased asked to call the unit or the community program if there are any questions about our services or eligibility.
