Description (Service) Provides assessment, education, treatment, and support for people with diabetes and pre-diabetes in two campus locations, West 5th and King St E.
Hours Mon-Fri 8 am-4 pm
Areas Served Brant County ; Burlington ; Haldimand County ; Hamilton (City of) ; Norfolk County ; St Catharines ; Welland ; Six Nations
Eligibility Ages: 55 year(s) and up 
 
Seniors 55+ who have been diagnosed with diabetes and kidney failure, are outpatients, are unable to adequately manage their foot care (as there is a higher risk of foot ulcer, with renal failure and diabetes), and are not able to pay for foot care services
Application A doctor's referral is required for one-on-one appointments. No referral needed to attend group education classes and the Pre-Diabetes Program. Call the Diabetes Program at King Campus.
