Description and Service Details
Description (Service) Provides assessment, education, treatment and support for people with diabetes, pre-diabetes, and people that are at risk of developing Type 2 diabetes. Also offers one-on-one or group consultation on monitoring blood sugar levels, medications, healthy eating, exercise, and other lifestyle choices. 
 
Group Education Classes include: 
* Living Well with Type 2 Diabetes 
* Carbohydrate Counting: Consistent Carbs 
* Label Reading 
* Eat Smart for Your Heart 
* Dining Out 
* Craving Change 
* Managing Insulin 
* Diabetes and Caring for Your Feet 
* Cutting Back on Sodium 
* Eating on a Budget 
 
Diabetes programs are offered at both the West 5th Campus and King Campus locations
Hours Mon-Fri 8 am-4:30 pm * Alternate Thursdays until 7 pm
Areas Served Brant County ; Burlington ; Haldimand County ; Hamilton (City of) ; Norfolk County ; Six Nations of the Grand River ; St Catharines ; Welland
Eligibility Ages: 18 year(s) and up 
 
Adults with pre-diabetes, type 1 diabetes and type 2 diabetetes
Application No referral needed to attend group education classes and the Pre-Diabetes Program * A doctor's referral or self referral is required for one-on-one appointments. Please bring the following to the appointment:  
* Current medications or list of current medications 
* Blood sugar meter 
* Log book of blood sugar readings 
* Your Health card
