Description and Service Details
Description (Service) Provides assessments, consultation and treatment to people with diabetes. Services are provided by Registered Chiropodists (licensed foot and ankle specialists) and Registered Practical Nurses. Satellite clinics are located in Stoney Creek (New Village Retirement Home), Burlington (Joseph Brant Community Health Centre), Haldimand, Norfolk, Brant, and Niagara (under the Feet First Niagara program).
Hours Health Centre Hours: Mon-Thurs 8am-8pm, Fri 8am-4pm

Eligibility People with diabetes who have risk factors such as peripheral neuropathy, decreased sensation, foot deformities, evidence of peripheral vascular disease, boney prominences, plantar callus, absent pedal pulses, (no open wounds) and/or barriers to using existing services (e.g. income, language)
