DescriptionFree in-home exercise program designed specifically for frail older adults by the Canadian Centre for Activity and Aging * 12-week program based on 10 gentle, progressive exercises that help seniors to maintain and/or improve functional mobility, increase activity and independence, and help prevent falls * individual is taught the exercises, supported, and monitored throughout the program by a trained PSW or Volunteer * requires no equipment. 
March of Dimes is the lead organization for the coordination of this program in the HNHB Local Health Integration Area * program is available through participating Personal Support Worker service providers, community support service agencies and volunteer programs.  
HoursOffice hours: Mon–Fri 9 am-4:30 pm
EligibilityAges: 65 year(s) and up 
and who: 
* live within the borders of the HNHB Hamilton-Niagara Haldimand-Brant LHIN (see 'Areas Served' above) 
* are ambulatory, using gait aid if necessary 
* are not receiving exercise instruction from a Physiotherapist; and  
* meet any of the following characteristics: frail, home-bound, declining mobility, at risk of falling, recently discharged from physiotherapy services and/or hospital, have difficulty performing activities of daily living, at risk of losing independence, sedentary lifestyle. 
* those in Long-Term Care are not eligible
