Description (Service) 60 bed long-term care home * residential care for 8 private rooms with private bathroom, 14 semi-private rooms with shared bathroom, and six basic rooms with shared bathroom * Special Care Unit with 14 beds for residents with Alzheimer's disease or other forms of dementia * 24-hour nursing and personal care * residents require own transportation or taxi services because of rural location

* Resident First Program: falls management * wound and skin care * pain and symptom management

* routine daily care by personal support workers/healthcare aides
