DescriptionA supportive housing project located in Burlington providing affordable, accessible housing with 24 hour attendant care in a group setting for 10 adults with physical disabilities age 16 years and up. 
 
Residence has 10 beds with individual rooms. Clients must be able to self-direct their attendant support needs and have their medical needs met by the Community.
HoursMon-Fri 9 am-5 pm
EligibilityAges: 16 year(s) and up 
 
Adults with a physical disability who use a wheelchair, and are able to direct or learn to direct their own care.
ApplicationSelf or family/caregiver/medical referral.
FeesProgram - rent; collective payment for food ; rent is 25% of annual income, eligible residents can apply for rent geared-to-income. * Fee assistance available for financially eligible residents.
