| Description | A multi-service organization supporting people of all ages in the development of a caring and responsive community. Community Support Programs: * information and referral regarding health and social service agencies, community groups and organizations, government resources * assistance and advocacy support to individuals and families who are experiencing difficulties, especially those who have no other place to go or those who are unable to access services without assistance * Income tax clinics for low income clients * Needs Coordination matches those who are in need of household items with those who are able to provide them * Transitional Housing is available for families who are homeless * rent is geared to income * a community support coordinator provides family support * Holiday Hampers provides hampers of children's clothing and toys for families who are experiencing financial difficulties Seniors Programs: Meals on Wheels: * hot nutritious meals at noon, Monday to Friday, to seniors and people with disabilities in Halton Hills Meals on Ice: * clients choose from a varied menu * frozen meals ordered and delivered weekly Wheels to Meals: * a social outing for seniors twice a month * transportation provided to a local restaurant or venue Congregate Dining: * provides a meal and social activities in seniors buildings in Halton Hills Home Maintenance and Repair: * provides help both inside and outside of the home to seniors and persons with disabilities * jobs could include yard maintenance, snow shoveling, painting, window washing, plumbing repairs, electrical repairs and seasonal housecleaning Telephone Assurance: * a scheduled phone call to ensure that all is well Friendly Visiting: * a community support service which brings volunteer "visitors" to the homes of seniors and persons with disabilities Home Help: * providing assistance with light housekeeping and meal preparation Letter Carriers Alert: * provides reassurance and a check on the well-being of those living alone who receive mail delivery to their door * a joint effort between Georgetown and Acton postal workers and Links2Care Client Intervention and Assistance: * provides support for seniors who may be at risk due to a critical change in their life situation * this may include homelessness, bereavement, or isolation * staff will meet with seniors and/or caregivers to offer information regarding programs and services * referrals may also be made to other agencies in an effort to create a sustainable plan, until the situation is stabilized * contact Seniors Coordinator at 905-844-0252 Supportive Housing Program: * program provides essential support services to eligible seniors living at Lakeview Villa in Acton * personal support workers are on site daily to assist residents with their personal care and activities * these may include bathing, dressing, medication reminders, light housekeeping and laundry * contact Anne, Support Services Coordinator 519-853-9916 Youth Programs: * Off the Wall (Acton) * Open Door (Georgetown) * Individual Youth Support available by phone and email Children's Programs: * Georgetown Parent-Child Centre Links2Kids School Age Programs * Acton Parent-Child Resource Centre Parenting Programs: * individual parenting support available * parenting programs available through the Parent Child Resources Centres | ||
